Through our Roadmaps to Take Action to Improve Health grant program, we awarded more than $1 million in grants to 28 community-based organizations to improve the social determinants of health.

In addition to grant funding, 30 organizations were provided with coaching, technical assistance, and peer-to-peer learning opportunities. This work is possible in part thanks to support from the Robert Wood Johnson Foundation and the County Health Rankings & Roadmap Program.
