Spokane Neighborhood Action Partners (SNAP) is Spokane County’s Community Action Agency and has been providing opportunities for people in need for over 50 years.  Located in Spokane Valley Washington, Riverwalk Point I & II provides over 100 units of affordable housing to households who are at 50% of the Area Median Income or below.  

In 2015 SNAP moved its housing portfolio to a no smoking indoors policy and will be evaluating whether there have been any health impacts for residents because of that change. They will also be evaluating the current health status of residents, how they access care and what types of health and wellness-related programming are important to them.

SNAP hopes to use the information gathered to inform their new partnership with Community Health Advocates to promote greater health and wellness of residents.

Initial Findings
Through surveys and interviews with residents, each organization participating in the Health Outcomes Demonstration Project collected their first round of data about the health outcomes of the program/s they are evaluating.  This snapshot reflects some initial findings which were shared with the cohort members at the November 2017 convening in San Francisco.  Organizations will be collecting their second round of data through the fall of 2018.  Check back in early 2019 for updates!
