NeighborWorks of Western Vermont is committed to sustainable homeownership for Vermonters, by helping residents purchase and maintain affordable, healthy, safe and efficient homes. Vermont homes are older, often historic farm or workforce housing that have seen less than ideal maintenance.

NeighborWorks of Western Vermont’s evaluation will focus on physical conditions of homes as they relate to asthma and COPD and to trips and falls or accessibility issues among handicapped and elderly. The organization is evaluating whether improvements to the physical conditions of a home result in improved health outcomes for patients whose conditions are triggered or caused by environmental rather than genetic issues.  

The organization hopes to develop an efficient close partnership with the medical community in order to support and deliver “doctor prescribed” physical improvements to homes of patients who are negatively affected by their living conditions.  NeighborWorks of Western Vermont hopes to prove that investing in physical improvements to achieve positive health outcomes is medically beneficial and cost effective, thus deserving of significant investment for better community health results.

Initial Findings
Through surveys and interviews with residents, each organization participating in the Health Outcomes Demonstration Project collected their first round of data about the health outcomes of the program/s they are evaluating.  This snapshot reflects some initial findings which were shared with the cohort members at the November 2017 convening in San Francisco.  Organizations will be collecting their second round of data through the fall of 2018.  Check back in early 2019 for updates!NeighborWorks Western Vermont stats
