Supportive housing
Community Housing Partnership's mission is to help homeless people secure housing and become self-sufficient. CHP provides direct services to formerly homeless individuals and families through the platform of supportive housing in high-need neighborhoods of San Francisco.
CHP is evaluating its Clinical Behavioral Health Services, through which formerly homeless single adults, seniors, and Transitional-Age Youth (TAY) who present with complex behavioral health issues (including those co-occurring with substance abuse) receive intensive clinical case management within their supportive housing residence.
CHP hopes to deepen its understanding about how the integration of clinical behavioral services into the supportive housing model impacts residents’ health outcomes, use of health services, and health access.

Initial Findings
Through surveys and interviews with residents, each organization participating in the Health Outcomes Demonstration Project collected their first round of data about the health outcomes of the program/s they are evaluating. This snapshot reflects some initial findings which were shared with the cohort members at the November 2017 convening in San Francisco. Organizations will be collecting their second round of data through the fall of 2018. Check back in early 2019 for updates!
